"Excellent" chance for me.

Apparently, you just need to fill in all the fields -- including the extra "Why should you choose me as a beta tester?" field -- to get excellent. If you skip the extra field, you get Above Average.

I was just reading through the site's javascript. I missed a field tho: "Please list any MMOGs you have BETA tested:". If you leave that blank, you'll only get Above Average.

So you need is to fill in the following for Excellent:

* all Personal Info
* your guild info
* at least 1 previously played game
* at least 1 previously played Elder Scrolls game
* at least 1 previously beta tested MMO
* at least 1 play style checked
* at least 1 preferred gaming activity checked
* DxDiag/OSX system information
* the extra "Why choose me" question answered

I doubt it changes anything in the long run (assuming it is just a graphical error), but I answered honestly and received an above average score. I checked the "am I part of the press or gaming industry" box, which took me to excellent. I unchecked it before submitting but I stayed at excellent.
